Fees for service will vary based on insurance coverage and income qualification. 

 

Insurance Submissions

Claims can be submitted to the following insurance payers:

 

Medical Mutual of Ohio

Aetna

Anthem Blue Cross Blue Shield

United Healthcare

Submission of claims does not guarantee coverage. Please make sure that all insurance information has been submitted at least 24 hours before your first appointment. If you are scheduled for a Telehealth appointment, please verify if your insurance company covers that type of service delivery.

We are always looking to expand on our insurance provider affiliations. Please email us if you do not see your insurance company listed.

If you have insurance that we are not affiliated with you may still be able to seek out of network reimbursement. We can provide documentation of session that you then send in to the insurance company for them to pay you directly.
